Where are you located?

Our address is 2718 Danbury St., San Antonio, TX 78217.

We are conveniently located off Nacogdoches, north of Loop 410 near the San Antonio International Airport. See the map below for more information!

Who Qualifies?

To apply for Grace Place, a person must:    

  • Have Alzheimer’s disease or dementia. They do not have to have a diagnosis from their doctor.
  • Be continent.
  • Be able to move safely and independently. 
  • Be able to feed themselves

Who does not qualify for Grace Place?

A person who poses a danger to themselves or others does not qualify for Grace Place. Examples include a person who:

  • Is physically violent.
  • Is verbally aggressive.
  • Lives alone. 
  • Needs medical care that exceeds our scope of work.

Do you provide transportation?

While we do not directly provide transportation, we are a stop on the VIAtrans Service.

What are your hours of operation?

The current hours of operation are 7:30 a.m. to 6:00 p.m. Monday through Friday.

What are your rates?

Grace Place offers a sliding fee for service to reflect our dedication to providing a reliable and affordable solution for families of loved ones with Alzheimer's/dementia. The fee structure includes three tiers, which families are eligible for based on the household size and the annual household income. Eligibility will be assessed upon admission and can be reassessed at the request of families if situations change.

Rates start at $1,000 for full-time and $600 for part-time and go up to $2,000 and $1,400, respectively. For a full breakdown, please click the link below.

What forms of payment does Grace Place accept?

Grace Place is a private pay facility. We accept the following as forms of payment for services:

  • Credit card
  • Debit card
  • Check
  • Money order
  • Cashier's check

 How do I make a payment?

You can conveniently pay online using the button below. This link will take you to SwipeSimple, a secure website, to process your payment.

For other payment methods, please get in touch with one of our team members at Grace Place Alzheimer's Center. We have also conveniently listed the payment methods accepted below.

Please note: Refunds are only granted in exceptional circumstances, within 30 days of the payment date, and are returned using the original payment method.  Some circumstances in which a refund might be granted include:

  • The payment was made in error.

  • The client did not authorize the payment.

Does Grace Place run promotions or offers?

While we do not have any current offers, we encourage families to check back often for any updates regarding promotions.

Is there an age requirement?

There is no age requirement, as long as the person meets all other eligibility criteria previously listed (see Who Qualifies? above).

Does Grace Place require a deposit?

Yes. A deposit equal to one month of service is required to complete admission. The deposit must be paid in full at the time of admission.

Can I bring my loved one for part of the day or certain days of the week?

You may request your preferred schedule (time of day) and which days of the week (minimum of 3 days) you would like your loved one to attend. Grace Place will attempt to accommodate your preferred schedule. However, all scheduling depends on availability within our licensed capacity.
